High School Team Camp Forms

Team Camp is July 14-16, 2012.  

Below are the 6 forms that each team will need to fill out. The Application and Waiver will be updated in 2012.  Each player needs an Application, the signed Waiver, and Proof of Physical-on any medical form, signed by a doctor. Please read all the information and make sure all forms are filled out legibly and completely. As you know, we only have 13 spots available for team camp and will accept teams based on completed packets on a first come, first serve basis.
