High School Team Camp Forms 

Session 1 is July 24-26 and session 2 is July 28-30.

Below are the 6 forms that each team will need to fill out. Each player needs an application, medical form, and Waiver.  Please read all the information and make sure all forms are filled out legibly and completely.  As you know, we have 26 spots available, 13 for each session of team camp and will accept teams on a first come, first serve basis.
